We beleive the more the more informed you are as a patient, the better experience you will have with our office.
We do not participate withh any insurance carriers. We will file your insurance as a courtesy for you in order to expedite the reimbursement process.
Insurance assignments are an agreement between you and the insurance carrier. As such, we do not accept insurance assignment. At the time services are rendered, your estimated percentage of the fee is required. As a courtesy to you, we will prepare all required forms and file them electronically to expedite the reimbursement process. Payment will be made to Marc Bowers, DMD, PA. If your insurance company does not reimburse the remaining balance, you are responsible for the payment. In order to ensure the remaining balance is received, a credit card pre-authorization form will be completed, granting Marc Bowers, DMD, PA permission to charge the remaining balance to your credit card. In the event the insurance company reimburses more than the estimated percentage, a refund will be issued to you and mailed to your address on file. If your insurance company chooses to pay in the form of a virtual credit card, an additional fee will be charged to your account. This amount will be 5% of the total insurance payment. To avoid this fee, you may contact your insurance company and request a check be mailed to our practice. Regardless of your insurance reimbursement schedule, any remaining balance is due in full within 45 days from the date of your initial treatment. Outstanding balances greater than 45 days will be subject to a 2% monthly finance charge (24% APR). If you do not have dental insurance, payment is due in full at time of service. Some services will require a deposit to schedule the appointment. This deposit will be non-refundable in the case of a failed appointment. A failed appointment is any appointment that is missed or cancelled without 24 hours notice. You should understand that if you do not show up for an appointment without 24 hours notice or present for an appointment without taking prescribed medications as directed by Dr. Bowers, you will be responsible for a $350 no-show fee.
